Same Day Access is the new way that assessments will be done at the MPNN CSB.
Instead of scheduling an assessment, you can simply walk in and get a Comprehensive Needs Assessment. Call or come in to one of our two counseling centers on the identified day/time below to get an assessment.
8:30 a.m. - 12:00 p.m.
Monday, Tuesday, Wednesday and Friday.
You or your child will receive a Comprehensive Needs Assessment and leave with a recommendation about the most appropriate, supportive services to address the issues which concern you. You will also be scheduled for a follow up appointment to begin services or be referred for a follow up appointment with appropriate supports or providers in the community.
What you need to bring:
Identification
Insurance Card
Social Security Number
Proof of income (will accept W2 or last 3 pay stubs)
List of medications and dosage (or bring medication bottles)
Any Custody or Legal guardianship Paperwork (if needed)
