Support Coordination is a comprehensive service that involves assessing an individual’s strengths and needs for services. Those with developmental disability may need help organizing and managing their lives, for example, arranging for housing, employment, health care, income, etc. Support Coordination addresses those every day needs and may also help with planning and coordinating total living needs. A Support Coordinator is assigned to each adult or child who requests and is found eligible for services. This individual coordinates resources and facilitates independence and stability.
